Criminal Disclosure


If you are applying for a course leading to working with children under 18, vulnerable adults, social services, uniformed services, teacher training or the health service you should ask at the interview for advice as certain convictions may affect future employment in these areas. All students need to be aware of their duty to inform the Safeguarding Team of any criminal convictions (spent and unspent).
Due to you answering YES to having previously had a conviction, you will need to complete a separate Confidential Disclosure Form. The college will be in touch to arrange this separately following the submission of this enrolment form.

Privacy Notice

How We Use Your Personal Information

This privacy notice is issued by the Education and Skills Funding Agency (ESFA), on behalf of the Secretary of State for the Department of Education (DfE). It is to inform learners how their personal information will be used by the DfE, the ESFA (an executive agency of the DfE) and any successor bodies to these organisations. For the purposes of relevant data protection legislation, the DfE is the data controller for personal data processed by the ESFA.

Your personal information is used by the DfE to exercise its functions and to meet its statutory responsibilities, including under the Apprenticeships, Skills, Children and Learning Act 2009 and to create and maintain a unique learner number (ULN) and a personal learning record (PLR). Your information will be securely destroyed after it is no longer required for these purposes.

Your information may be shared with third parties for education, training, employment and well-being related purposes, including for research. This will only take place where the law allows it and the sharing is in compliance with the data protection legislation.

The English European Social Fund (ESF) Managing Authority (or agents acting on its behalf) may contact you in order for them to carry out research and evaluation to inform the effectiveness of training.

All personal data will be held in accordance with current Data Protection legislation. A copy of the College’s Register Entry is held in the Library for reference. I understand that the information provided on this form may be processed for purposes connected with the College as an educational institution.

We may allow access to our software supplier for the purpose of fault finding and development. We have written assurance from the supplier that they will only use the data for this purpose, will protect it and will remove such data from their systems once they have completed their work. Attendance and Progress information may be passed to parents/guardians/employers. The College is obliged to provide Job Centre Plus access to the Attendance Register and Payment Claim records for Job Centre Plus learners.

Declaration


You have the right to cancel your enrolment and receive a full refund of any fees paid if we receive written notice up to seven working days from the day after that of your enrolment. However, if after commencement of the course you wish to withdraw and receive a refund this may only be granted on production of a medical certificate.

You should put your request in writing to the Head of Information Services, Southport College, Mornington Road, Southport PR9 0TT or by email to ism@southport-college.ac.uk. You will be refunded with the full amount of the transaction within 10 working days.
